Physical Therapist Traveler             **SIGN ON BONUS**

Position:  Physical Therapist Traveler
Hours:  Full-Time

Where:  All Clinical Settings
Location:  Midwest Region, Illinois

If you want to use your career as a way to explore interesting new places, a physical therapy travel job may be right for you. As a Physical Therapist Traveler you have the opportunity to work in a variety of locations throughout the Chicagoland area. This position will provide opportunities to earn extra money, sign-on bonus, and relocation allowances.

Job Description

  • Obtain physicians’ prescriptions and consent for evaluation, in conjunction with government and/or facility policy
  • Conduct initial screenings, diagnostic assessments and clinical observations of patients
  • Determine patient’s appropriateness for therapy, assessing cognitive fitness for learning and physical capability
  • Assess severity of patient’s condition: Determine if skilled therapy or restorative programs are indicated
  • Develop treatment plan, setting goals that are attainable for patient progress in collaboration with team of therapists, facility staff and family members
  • Determine patient’s level of function and need for continued therapy or recommendation for discontinuation of therapy services


Requirements

  • Bachelor’s, Master’s, or Doctorate  Degree, as a Physical Therapist from an accredited college or university
  • Current Illinois License as a Physical Therapist

Occupational Therapist – OT
Position:  Occupational Therapist – OT
Hours:  Full-Time
Where:  Various Senior Living Communities

Location:  Northwest Suburbs, Illinois

Job Description

  • Guide patients in use of therapeutic exercises and activities, according to treatment plan, or supervise COTA in administering the treatment plan
  • Provide therapeutic intervention to optimize functional skills for fine motor, visual motor, visual perception, bilateral coordination and sensory processing for optimal performance
  • Instruct patients in adaptive techniques and use of adaptive equipment for functional skills (e.g. dressing, feeding, mobility and other skills of daily living as relevant to particular patient)
  • Provide ADL instruction and techniques
  • Assess the need for and make recommendations for assistive devices, following up as needed
  • Provide facility staff and family with demonstrated and/or written instruction on therapeutic and ADL methods
  • Provide education and/or home program to promote functional and ADL skills, as appropriate


Requirements 

  • Bachelor’s, Master’s, or Doctorate  Degree, as an Occupational Therapist from an accredited college or university
  • Current Illinois License as an Occupational Therapist

Social Worker - PRN
Position:  Social Worker - PRN
Hours:  32-40 hours/month
Where:  SNF's 
Locations:  Peru/LaSalle, Illinois

Job Description

  • Ensure that the facilities’ social service (SS) programs are meeting residents’ needs according to state/federal regulations and provide guidance towards meeting and exceeding them
  • Contact state and federal agencies with questions, concerns, interpretations, etc. regarding regulations on social service programs as needed
  • Maintain awareness of changes in government regulations, including OBRA, Illinois Department of Public Health, Veteran’s Administration as well as Joint Commission on Healthcare Organization standards
  • Review documentation, including care plans, progress notes and policies & procedures, for compliance and to ensure optimal reimbursement, making suggestions for changes and/or improvements as appropriate
  • Examine MDS to identify key areas of relevance for social services care planning
  • Conduct social service assessments as needed, observing and interviewing both social services staff and residents for the purpose of appropriate social services planning and implementation
  • Identify training needs and provide instruction to appropriate staff on issues pertaining to social services and regulatory updates
  • Promote and develop communication and partnership between facility departments to address quality of life and dignity issues of the residents


Requirements

  • Must hold an Illinois license to practice Social Work
  • Prior experience in a nursing home preferred
  • In-depth knowledge of state/federal regulations pertaining to social service programs
  • Ability to develop programming with a multi-disciplinary approach
  • Ability to interpret maladaptive behavior among residents in terms of its potential root cause and remedial social service intervention
  • Ability to network among facility staff and leadership, establishing rapport to facilitate cooperation with consultation efforts and suggestions
  • Assertiveness: Ability to advance one’s ideas and observations and suggest new courses of action
  • Creative problem-solving capabilities
  • Excellent follow-through, organizational skills and time-management
